Welcome to Hope Youth!

We are asking parents and guardians to register their youth for participating in our youth program. Whether you are visiting or a regular attendee. This will ensure we have up-to-date information and the necessary consent for your youth to participate in all Hope Youth activities, including outdoor excursions, retreats, camps, and social events. This will also ensure proper resource allocation for the ministry.

Hope Bible Church Markham is collecting and retaining this personal information for the purpose of enrolling your child in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our Church. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Hope Bible Church Markham to limit the information collected, or to view your child’s information, please contact us. We require permission from parents/guardians for each youth participating in Hope Youth to complete this form in its entirety. Information received is confidential and is being gathered for the purposes of serving your child while in the care of Hope Bible Church Markham. Any medical information collected here serves to authorize Hope Bible Church Markham, and its staff and volunteers, to obtain medical assistance in emergencies.
